Artificial intelligence (AI) is transforming the practice of medicine by reshaping how physicians diagnose, plan treatment, communicate with patients, and manage information. This review synthesizes evidence from recent clinical trials, systematic reviews, and policy guidance to evaluate the changing role of physicians in an AI-driven era. Current AI applications—ranging from imaging interpretation and predictive analytics to ambient documentation and decision support—demonstrate measurable benefits in efficiency and diagnostic accuracy, yet they introduce new ethical, technical, and professional complexities. Physicians are evolving from sole decision-makers to human-in-the-loop supervisors who interpret, validate, and contextualize algorithmic outputs. The article outlines the competencies required for this transition—AI literacy, data governance awareness, and critical evaluation of bias and interpretability—and reviews emerging educational frameworks that embed these skills into undergraduate and postgraduate curricula. Ethical and legal analysis reveals the need for shared accountability among clinicians, institutions, and developers, along with transparent communication to sustain patient trust. Persistent challenges include technical limitations, workflow misalignment, and equity gaps in low- and middle-income settings. The paper concludes with forward-looking recommendations for physicians, health-care organizations, and policymakers to foster safe, equitable, and outcome-driven integration of AI in clinical care, emphasizing research priorities such as long-term patient outcomes, bias mitigation, and lifecycle monitoring of adaptive models.
